Requirements
  • Original and two photocopies of:
    • Birth Certificate
    • Baptismal Certificate
    • Marriage Contract of the parents under Catholic rites
  • Two (2) recent ID pictures (2x2 black and white or colored)
  • Photocopy of the Report Card bearing signed by the Principal with latest grades and must have an average grade of at least 80% both in Academics and Conduct.
  • Certificate of Good Moral Character signed by the Principal of the school last attended.
  • Medical records/certificate – history of past and present illnesses including vaccination records of the child. (After the applicant has passed the Entrance Examination)
  • If alien, Study Permit/ Unified photocopy of Alien Certification Registration (ACR) is needed.

Registration Procedure
  • Fill up the Application Form and attach the complete requirements.
  • Proceed to the Guidance Office for the interview and verification.
  • Proceed to the Cashier for the payment.
  • Submit Registration Form and receipt at the Registrar’s Office for the issuance of the Test Permit.

Fees
  • Testing and Application Fees – Php 400.00 (non-refundable)
  • Reservation Fee for those who passed the Entrance Examination – Php 1,000.00/ applicant (non-refundable)

Reminders
  • Kinder applicants should be 4 ˝ to 5 years old and preferably have attended nursery class.
  • Prep applicants should be 5 ˝ to 6 years old by June 2008 and preferably have attended nursery and kinder classes.
  • Take the Entrance Examination as scheduled. There will be no rescheduling of the test.
  • Bring your TEST PERMIT and #2 Mongol pencils with eraser on the day of the examination.
  • Come on schedule for the release of test results.
  • Any misinformation given will disqualify any applicant from enrolling/staying in SJA.
